Bleach
It looks more like a gas cap to me.

Dave Henderson
Looks like a rad cap to me. I've seen others similar, with one ear. Radiator caps get rusty underneath, gas caps ordinarily don't.
Larry Schramm
Is the square hole in the center a vent hole? If it is, then I still think it is a gas cap. Edited September 17, 2015 by Larry Schramm (see edit history)
Bleach
If it were a radiator cap, I'd think the part in contact with water would be made or coated with something not prone rusting like the outside of this cap.I've seen plenty of rusty gas caps. Curti
I vote aftermarket radiator cap. The square hole on the underside is possibly for an accessory mount.
Larry Schramm
I am going to revise my guess and I am going to say that it is an oil fill cap that fits on the valve cover for vehicles prior to about 1980. Here is a picture of an oil fill cap from a small block Chevy '70's vintage. Others were similar.

If it goes on a valve cover it needs a raised portion to clear.
